Luận văn Đánh giá đặc điểm lâm sàng và kết quả phẫu thuật tạo hình van hai lá tại Bệnh viện tim Hà Nội. Hở van hai lá là một bệnh hay gặp trong các bệnh lý tim mạch, với thương tổn liên quan đến mô van, vòng van, bộ máy dưới van hay phối hợp. Tỷ lệ khá phổ biến từ 5-24% trong tổng số các bệnh lý tim mạch [1]. Hở hai lá thường nhanh chóng dẫn đến suy tim và nhiều biến chứng khác nếu không được sửa chữa kịp thời, nếu điều trị đúng có thể chữa khỏi hoặc kéo dài thời gian sống cho bệnh nhân.

Hình thái thương tổn của van hai lá trong bệnh van tim mắc phải hoàn toàn tùy thuộc vào nguyên nhân gây bệnh, nhưng tựu chung lại gồm ba thể bệnh chính là: Hẹp van đơn thuần, hở – hẹp van và hở đơn thuần [1]. Siêu âm tim Doppler màu là biện pháp thăm dò hình ảnh quan trọng nhất giúp ích cho lâm sàng trong việc chẩn đoán bệnh, thể bệnh và đưa ra chỉ định điều trị [2].
Có nhiều phương pháp đã được nghiên cứu điều trị hở van hai lá, tuy nhiên ngoại khoa có ưu điểm nổi bật với những kỹ thuật tiên tiến từ thập kỷ 60 thế kỷ 20 trở lại đây [3].
Phẫu thuật tim hở điều trị hở van hai lá bao gồm hai loại là: Thay van và tạo hình van. Phẫu thuật thay van hai lá bằng van nhân tạo tuy rất phổ biến do kỹ thuật không quá phức tạp và có thể sử dụng cho mọi loại thương tổn van nhưng cũng có một số hạn chế như: Tỷ lệ biến chứng huyết khối sau mổ cao (3-5%) đối với van cơ học, chất lượng sống giảm (do dùng thuốc chống đông máu, suy tim) chi phí phẫu thuật lớn. Phẫu thuật tạo hình van do bảo tồn được bộ máy van hai lá, cho phép khắc phục được một phần các hạn chế nêu trên, đem lại cho bệnh nhân một cuộc sống tương đối bình thường, với kết quả tốt, hậu phẫu ngắn, ít biến chứng, nên là phương pháp điều trị được ưu tiên lựa chọn cho những trường hợp cấu trúc van không bị hỏng quá nặng [4],[5]. Tuy nhiên, do t ạo hình van hai lá là một kỹ thuật phức tạp, đặc biệt đối với thương tổn van trong thấp tim nên cũng ít phổ biến và phát triển không đồng đều ở các trung tâm mổ tim trên thế giới cũng như trong nước [6].
Tại Việt Nam phẫu thuật tạo hình van hai lá được phát triển mạnh từ đầu những năm 90 của thế kỷ trước, dẫn đầu là tại Viện tim Thành phố Hồ Chí Minh. Cho đến nay đã đúc kết được một số kinh nghiệm, đặc biệt đối với tổn thương van do thấp, cho những kết quả ban đầu rất khả quan. Tại Bệnh viện tim Hà Nội, một bệnh viện non trẻ mới đi vào hoạt động trong khoảng hơn chục năm nay, loại phẫu thuật này đã được đưa vào thường quy. Tuy tất cả các trung tâm mổ tim trong nước đều tiến hành tạo hình van hai lá dựa trên kỹ thuật tạo cơ bản theo trường phái Carpentier nhưng tùy theo kinh nghiệm và sự sáng tạo trong quá trình phẫu thuật mà kết quả sớm cũng như lâu dài là không giống nhau [7],[8].
Trước một bệnh nhân hở van hai lá, người thầy thuốc cần cân nhắc cẩn trọng chỉ định phẫu thuật đúng thời điểm để người bệnh tránh được biến chứng gần và xa, đem lại kết quả sau mổ tốt nhất [9]. Các khám nghiệm lâm sàng và siêu âm tim là biện pháp tin cậy để bác sĩ đưa ra chỉ định phẫu thuật đúng đắn. Việc nghiên cứu nhằm đánh giá lại những đặc điểm của từng kỹ thuật và kết quả chung – dài hạn sau mổ sẽ rất có ý nghĩa trong việc góp phần nâng cao chất lượng phẫu thuật tạo hình van hai lá, một trong những mũi nhọn của phẫu thuật tim trong nước hiện nay.
Xuất phát từ những đòi hỏi của thực tiễn chúng tôi tiến hành đề tài “Đánh giá đặc điểm lâm sàng và kết quả phẫu thuật tạo hình van hai lá tại Bệnh viện tim Hà Nội” với hai mục tiêu sau:
1.    Mô tả đặc điểm bệnh van hai lá và kỹ thuật tạo hình van.
2.    Đánh giá kết quả tạo hình van hai lá tại Bệnh viện tim Hà Nội.
