Luận án Nghiên cứu đánh giá kết quả điều trị phẫu thuật thông vách nhĩ thất bán phần. Thông vách nhĩ thất bán phần (partial atrioventricular septal defect – pAVSD) chiếm 80% tổng số bệnh thông vách nhĩ thất. Đây là bệnh tim bẩm sinh có tần suất tương đối thấp, chiếm tỉ lệ 20% tổng số bệnh thông liên nhĩ. Bệnh tim bẩm sinh thông liên nhĩ đứng hàng thứ tư sau thông liên thất, còn ống động mạch và tứ chứng Fallot [5]. Tổn thương giải phẫu cơ bản thông vách nhĩ thất bán phần gồm: thông liên nhĩ lỗ nguyên phát, hai bộ máy van nhĩ thất riêng biệt nằm trên một mặt phẳng, van hai lá (van nhĩ thất trái) có khe lá trước van hai lá có thể gây hở van, van 3 lá (van nhĩ thất phải) có thể hở van do khoảng trống giữa lá vách và lá trước van ba lá rộng. Trên thế giới, bệnh được gọi với nhiều thuật ngữ khác nhau như: thông sàn nhĩ thất bán phần hay kênh nhĩ thất bán phần (pAVC: partial atrioventricular canal), thông liên nhĩ lỗ tiên phát (ostium primum atrial septal defects), khiếm khuyết gối nội mạc (endocardial cushion defects) [5],[6],[7],[16],[18],[23],[66],[77], [83],[98].

Nghiên cứu đánh giá kết quả điều trị phẫu thuật thông vách nhĩ thất bán phần Thông vách nhĩ thất được mô tả lần đầu tiên bởi Maude Abbott (1936) với 2 thể thông liên nhĩ lỗ tiên phát và thông vách nhĩ thất toàn phần. Roger và Edwards (1948) mô tả những nét tương đồng về tổn thương của hai thể này, sau đó Wakai và Edwards (1958) thống nhất gọi 2 thể bệnh này là thông vách nhĩ thất bán phần và thông vách nhĩ thất toàn phần theo thương tổn giải phẫu đặc trưng. Theo Kirlin (2013) [54], các tác giả Lev và Bharati mô tả chi tiết nút nhĩ thất, bó His trong bệnh lý thông vách nhĩ thất, đồng thời đưa ra khái niệm thông vách nhĩ thất thể trung gian (intermediate atrioventricular septal defect). Tuy nhiên mãi tới năm 1979 Gian Piero Piccoli và cộng sự (cs) nghiên cứu trên 114 ca thông vách nhĩ thất mới mô tả một cách chi tiết về vòng van, vách liên thất buồng nhận, sự dài hơn bình thường của đường thoát thất trái và sự thiểu sản các lá van [83].
Lillehei và cs (1954) phẫu thuật thành công trường hợp thông vách nhĩ thất đầu tiên, tiếp sau đó là Kirklin và cs (1955) mặc dù tỉ lệ tử vong khi đó lên tới 50%. Các biến chứng thường gặp sau phẫu thuật là block nhĩ thất hoàn toàn, hở van hai lá nặng, hẹp đường ra thất trái [55],[56],[60],[61],[88]. Trên thế giới, thông vách nhĩ thất đã được nghiên cứu từ khá lâu và đã có nhiều tiến bộ, tuy nhiên cũng còn những vấn đề chưa được thống nhất giữa các nhóm tác giả. Ví dụ như: đóng khe lá van nhĩ thất trái cho tất cả các bệnh nhân hay không khi sửa van hai lá? Stephan Aubert và cs (1974 – 2001) đã đóng khe van hai lá cho tất cả các bệnh nhân nghiên cứu (208 bệnh nhân)[15], El-Najdawi (2000) chỉ đóng khe van hai lá cho những trường hợp hở vừa và nặng, không đóng khe van hai lá ở các trường hợp không hở hoặc hở nhẹ trước mổ với quan điểm không hở thì không đóng và không cần thiết phải đóng toàn bộ chiều cao khe van nếu như van đã kín [34]. Vấn đề vá thông liên nhĩ như thế nào để tránh gây tổn thương nút nhĩ thất và bó His khi giải phẫu hệ thần kinh tim trong bệnh lý này có nhiều thay đổi so với bình thường. Leca, Neveux, Vouhe đề xuất kỹ thuật vá thông liên nhĩ để xoang vành sang trái sẽ giảm được tối đa nguy cơ trên [7]. Mavroudis C và Backer C.L. (2003) lại cho rằng dù để xoang vành qua phải hay qua trái cũng không cải thiện tỷ lệ biến chứng về dẫn truyền của tim [66].
Tại Việt nam, các trung tâm tim mạch như Bệnh viện Việt Đức, Viện Tim Thành phố Hồ Chí Minh, Bệnh viện tim Hà nội, Trung tâm tim mạch Bệnh viện E, Bệnh viện Trung ương Huế đã phẫu thuật thành công những trường hợp thông vách nhĩ thất bán phần đầu tiên. Tuy nhiên, chưa có tác giả nào báo cáo đầy đủ về giải phẫu, sinh lý cũng như chẩn đoán và điều trị phẫu thuật bệnh lý này. Vì vậy, chúng tôi tiến hành nghiên cứu đề tài “Nghiên cứu đánh giá kết quả điều trị phẫu thuật thông vách nhĩ thất bán phần” nhằm mục tiêu:
1. Nhận xét đặc điểm lâm sàng, cận lâm sàng thông vách nhĩ thất bán phần được phẫu thuật tại Bệnh viện Tim Hà Nội.
2. Đánh giá kết quả phẫu thuật điều trị thông vách nhĩ thất bán phần tại Bệnh viện Tim Hà Nội.
